Whether a treatment plan, progress note, service or consent, capturing patient signatures for vital documentation can be challenging especially when care is provided remotely. One solution to this problem is to deploy a patient portal. That comes with some challenges in both cost and complexity…assuming the given platform even supports capturing remote signatures. It also comes with support overhead.

Fivebase's remote patient signature app is designed to bridge the gap between manual processes with multiple steps and having to deploy and support a complex portal. It also helps in eliminating on going costs of 3rd party e-signature platforms most often used in manual processes. Our solution will work with most EHR's that offer both read and write API access (Eg, Netsmart's myAvatar, Cantata's Arize, Epic, Nextgen, etc.).

The flow is simple: A special / unique link is sent via SMS (text) or email from your EHR to your client asking for sign off on one or many documents. They then “soft” authenticate as a secondary verification step with an optional third 2FA step. Once validated, they are presented with an inbox of documents that need to be signed. When signed and submitted, a document is either transmitted back to your EHR as a chart document and / or discrete data. Other post signature options include sending notifications to key staff that a document was signed.
